Annual Health Reviews

We offer an Annual Health Review if you have been diagnosed with

  • an Intellectual Disability (commonly known as a Learning Disability) or
  • an Autistic Spectrum Disorder (ASD) 

Annual Health Check Easy Read Guide.

An Annual Health Check is done by your practice nurse once a year. Your nurse will take a closer look at how you are doing – making sure you’re healthy. Sometimes you will also see a doctor.

  • You don’t need to be ill to get a health check, but you can ask your doctor about anything that is hurting or worrying you. 
  • The nurse will ask you some questions about your health.
  • The nurse will also check some things, like your weight, height and blood pressure.
  • It is a good way to get to know your nurse better, and for them to find out more about you.
  • Your usual GP will look at all the results and will arrange a doctors appointment only if it is necessary.
  • If you mention something during the appointment that needs to be assessed by a doctor, the nurse may ask you to come back another time to see your usual GP.

If your Intellectual Disability or ASD is very mild or does not impair your functioning in relation to health matters, you or your GP may consider you unsuitable for an Annual Health Review and exempt you from being invited every year.

Preparing for your Annual Health Check

Please prepare for your health check by filling in a questionnaire. If you have a carer, they will usually be asked by the practice to help you with this.
