Diabetes Management at Camphill Medical Practice
If you have diabetes, you will be invited for a review appointment with a nurse at least once a year. Depending on your last blood test results and the need for changes to your medication, you may be invited for review more often, or may also have appointments with a GP to discuss your treatment.
At least once a year, the nurse will
- Discuss your diet, exercise and lifestyle
- Check your blood pressure, weight and height
- Discuss and support you with your self-management (follow link for self-help information)
- Discuss your foot health and how to check your feet (follow link for self-help information)
- Discuss your medicines and how you take them
- Check your bloods including your HbA1c
- Consider your psychological well being and the need for psychological wellbeing support (follow link for self-help information)
- Consider relevant educational programs and other supports
This may be done over more than one appointment, for example an appointment for blood tests and measurements, and another appointment for foot checks, review of the results and agreement of the plan going forward.
We highly recommend that you register with the My Diabetes My Way Website. It has lots of useful information and allows you to view your own up-to-date diabetes clinic results, so that you have a chance to reflect on the results prior to your review appointment, to help you to manage your condition more effectively.
